total order is $30.01 to $50.00

total order is $50.01 to $100.00

total order is $100.01 to $200.00

total order is over $200.00
SALES TAX: We must charge sales tax on all orders within the state of Nebraska (NE). No sales tax will be charged to orders from other locations, or tax exempt organizations. (Please provide tax exempt certificate with order.)
ORDERING BY FAX: Make a copy and complete the order form found at
Order Form, and fax it to 402-491-3221. Make sure that you have included your credit card number. (There is no need to include a cover page with your fax.)
